    Please note the following:
    a) This initiative is applicable for AVA’s GIRO customers only. Please do not use this form if you are not a GIRO customer.
    b) Please provide the customer reference number as stated in the top right hand corner of your monthly GIRO statement.
    c) We will inform you of the status of your application within 5 working days by email. The hardcopy statements will be discontinued once your

    I hereby agree to receive monthly GIRO statements electronically from AVA via this email address. This authorization will be in force unless terminated upon your receipt of my written revocation. I also agree that I will not be receiving any paper GIRO statements once my
     application is successful.
